[COMPANY NAME] SHIFT SWAP REQUEST ============================================================ This request must be submitted at least 48 hours before the earlier of the two shifts. Both employees must sign. An unapproved swap leaves the originally rostered employee responsible for the shift. A shift swap is only a swap once it is approved. Until then the originally rostered employee is still responsible, which is exactly what gets disputed when something goes wrong on the shift.
The confirmation block is the part most forms omit: the person taking the shift confirming they are qualified for it and that it will not breach rest requirements or maximum hours.
Frequently asked questions
- Do shift swaps need manager approval?
- Yes. Until a swap is approved and the roster updated, the originally rostered employee remains responsible for the shift. That is the whole reason the form exists.
- What should be checked before approving a shift swap?
- That the person taking the shift is qualified and authorised for it, that it does not breach the minimum rest between shifts, and that it does not take them over the daily or weekly hours limit — or create overtime nobody budgeted for.
