Record times on a 24-hour clock. Break is in minutes. Hours are decimal —
seven and a half hours is 7.5, not 7.30.

Employee signature: [Signature] Date: [Date]
Manager approval: [Signature] Date: [Date]

A timesheet is only useful if it is signed by the person who did the work and approved by the person who can vouch for it. Both lines are on this one.

Pick the period and the dates fill in automatically. Add a project column if you bill or cost by project.

Frequently asked questions

What should a timesheet include?
The employee's name and identifier, the period, a dated row for each day with clock-in, clock-out, break and total hours, a period total, and signature lines for the employee and the approving manager.
How long should timesheets be kept?
Wage and attendance registers are required to be maintained under the Code on Wages, and record-retention periods are prescribed by the applicable rules. Three years is a common floor in practice; check the rules for your state and establishment type.
Should hours be recorded in decimal?
Yes, for payroll. Seven and a half hours is 7.5 — writing 7.30 is the commonest timesheet error and it under-reports by twelve minutes every time.
