Different feedback needs different questions. What you ask a departing employee is not what you ask someone four weeks into the job, and a generic form gets generic answers from both.

- What should an exit interview ask?
- What prompted the search, what the deciding factor was, what would have made them stay, what you should keep doing, what the next person should know, and whether they would recommend the company or return. The second and third are the ones that change anything.
- Is exit interview feedback reliable?
- Individually, not always — people soften it to protect a reference. In aggregate across a quarter it is among the most reliable signal an organisation has, because the same themes recur regardless of how carefully each person phrased them.
- Should manager feedback be anonymous?
- In a small team, anonymity is often illusory and everyone knows it. What matters more is whether managers respond well to feedback, which is a cultural question the form cannot solve.
