POLICY ACKNOWLEDGEMENT

============================================================

Employee: [Name] Employee number: [Number]
Department: [Department]
Occasion: On joining

I confirm that I have been given access to the policies listed below, that I have
read and understood them, that I have had the opportunity to ask questions about
them, and that I agree to comply with them.

   Policy Version Initials
   ------------------------------------------------------------------
   Code of conduct
Policy on prevention of sexual harassment at the workplace (POSH)
   Leave policy
   Attendance policy
Information security and acceptable use policy
   Expense and reimbursement policy
   Grievance policy
   ------------------------------------------------------------------

I understand that these policies may be amended from time to time, that I will be
notified of material changes, and that the current version is the one that applies.

I understand that a breach of these policies may result in disciplinary action.

For the POSH policy specifically, I confirm that I have been told who the members
of the Internal Committee are and how to make a complaint.

A policy nobody has acknowledged is difficult to enforce. This records which policies, at which version, on what date — the version column being the part that is usually missing when it matters.

The POSH line is separate on purpose. The Act expects employees to know who the Internal Committee members are and how to complain, not merely that a policy exists somewhere.

Frequently asked questions

Why do employees need to acknowledge HR policies?
Because a policy nobody has been shown is hard to enforce. A signed acknowledgement, with the version and date, is what makes a policy usable in a disciplinary process.
Should policies be re-acknowledged when they change?
Materially changed ones, yes. That is why the version column exists — an acknowledgement of version 1.0 says nothing about version 2.0, and a blanket 'policies as amended' clause is weaker than a fresh signature.
What should a POSH acknowledgement specifically confirm?
That the employee has been told who the Internal Committee members are and how to make a complaint — not just that a policy exists. The Act expects employees to know the route, and displaying it conspicuously is a separate obligation.
